*Immunizations: Wisconsin state law (252.04) requires that all students in grades K-12 be immunized against diphtheria, tetanus, pertussis (whooping cough), polio, measles, and rubella. Written evidence of completed basic and booster immunizations, including the day, month, and the year is required. This requirement will be waived for an individual student only for health, religious, or personal conviction reasons. If students are not fully immunized upon admittance to the school, they must have received at least one dose of any vaccine required for their grade within 30 days of admission. They must remain on schedule, and complete all required immunizations within one year. For more information please see the Student Immunization Law Age/Grade Requirements.
